Equipment Discrepancy Report (EDR)
This mishap report is to be completed by the Lead foreman for any issues that happen on the job site, transportation, pre and post shift set up for those crew members under their charge. The mishap report shall then be routed up the chain of command through the Division Manager. This mishap report will then be logged by Staff management to create trend analysis to better train employees for common reoccurring mishaps on job sites. Please report all mishaps within the individual crews and or divisions no matter the capacity of the mishap in order to supply management with issues that we can develop further training on or lead to terminating unsafe practices on job sites and certain conditions present.
