Crew Designation Log

The use of this form is to designate crews within each division daily by the managers of the division. This completed document will be logged by both operations and administration manager for pay changes to present employees as follows:

If any employee on a designated crew is absent the present employees of that crew will receive the daily rate of pay of this absent except for cases of:

(Scheduled PTO & Scheduled UPTO)

It is the division managers responsibility to ensure crews are sufficiently staffed in times of scheduled time of all employees.
